Dear Parents and Guardians,
On Thursday 30th November 2023, we will be hosting a team from the Centre for Health Protection (CHP) for the Seasonal Influenza Vaccination Clinic. This outreach programme is free of charge and will be held during school hours.
In the following days, your child will be sent home with two documents:
- Consent/Refusal Form
- Invitation for eHealth Registration
We kindly request your cooperation in completing the relevant documents and returning them to us, regardless of whether you select for your child to be vaccinated or not. To avoid any errors, please read the documents carefully and ensure that all of the necessary sections are completed. We request that the forms are returned to the medical rooms by the 3rd of October.
If you have multiple children in the school, please complete a separate form for each child.
