Forms

When a student is allocated a school owned device they are required to sign one of following agreement forms.



DBIS Student Bring Your Own Device Agreement

Students joining DBIS in Year 7 to 9 have a choice as to whether they bring their own laptop or Chromebook to school or purchase a Chromebook from the DBIS pre-purchased stock.

Students in Year 12 and Year 13 are required to bring their own laptop to school.
It is also strongly recommended that Students in Year 10 and 11 bring a laptop.

Before the laptop can be connected to the school network, students should complete this agreement.

DBIS Chromebook Loan Form (for online learning)

During periods of closure, students in Years 3-6 are allowed to take their school Chromebook home.  Parents and students are required to sign the Chromebook agreement before the device is given to the student.

Acceptable Use Policy Form

Students entering Secondary are required to complete the school's acceptable use policy to ensure they understand what the school's expectations are for the appropriate use of technology at DBIS.
