Department of Health’s School Student Health Service (Year 7)

Please read carefully the Department of Health’s invitation to join the Student Health Service of which can be found HERE on the Hub.  This is not a DBIS initiative but parents may wish to take advantage of the offer for Year 7 students.

In view of the latest development of COVID-19, the Government health centres need to adopt social distancing and implement infection control measures, they can only offer health assessment for the Secondary Year 7 students. If the situation improves, they would consider arranging health assessment for other grades.

Each enrolled student will be scheduled for an appointment for health assessment at The Western Student Health Service Centre, 9/F Rumsey Street Multi-storey Carpark Building, 2 Rumsey Street, Sheung Wan, Hong Kong. The appointment times for will be arranged outside of school hours from November 2020 to October 2021 and parents will be responsible for organising their own transport arrangements.

The Student Health Service is provided free of charge to students who are HK residents or other eligible persons as noted on the form.

Please refer to the attached Notes for Parents/Guardians regarding guidelines to verification of eligibility.  For non-HK residents, the annual fee of HK$535 will have to be paid on the appointment day.

Please return the completed application form to the School Office on or before 10 September.

Hard copies of the application form can also be obtained from the school office.
