Dear Parents of Year 11, 12 and 13 Students,
The Department of Health (DH) has invited secondary schools to participate in the Human Papillomavirus (HPV) Vaccination Catch-up Programme. This programme is for current Year 11–13 female students who have not yet received two doses of the HPV vaccine. If you’re interested for your child to receive the vaccine, please complete the Jotform here.
