[Medical] Seasonal Influenza Vaccination

2025/26 Seasonal Influenza Vaccination School Outreach

 

Dear Parents and Guardians,

 

We have joined the Seasonal Influenza Vaccination School Outreach (free of charge) programme organised by the Department of Health. Dedicare Medical Centre will come to our school on Monday 6th October 2025 to provide the influenza vaccination to our students. 

If your child is at our EYFS campus, please collect them after school at 2.40pm and take them up to the Globe at our Main Campus for their vaccination.  

In order to verify the student’s eligibility for this programme, parents must return the following documents on or before Monday 15th Sept 2025:

 

  1. The signed and completed consent form; and
  2. A copy of ONE of the following documents of the student:
    1. Hong Kong identity card / Hong Kong birth certificate
    2. Hong Kong re-entry permit,
    3. One-way permit / two-way permit;
    4. Identity document for visa purpose; or
    5. Foreign passport – showing pages with (i) identity information and travel document number, and (ii) valid visa

Kindly pay attention to the following:

  1. Before completing the consent form, please read the instructions carefully on page 2 of the consent form.
  2. All participating students must return documents listed (1) and (2) above. If there is any missing or incomplete document, Dedicare Medical Centre will NOT provide vaccination service to the student.

For any other inquiries about the vaccination (for instance, any medical-related questions and whether the student is suitable for vaccination), please consult your family doctor or contact Dedicare Medical Centre at 2468 2248.
