Dear Parents and Guardians,
This is a reminder for all Year 6 parents about the upcoming DTap (Diphtheria, Tetanus and Pertussis) vaccination clinic.
The clinic will take place on Tuesday 28th October.
The consent/refusal forms that were sent home last week must be completed and returned to school by this Friday, 17th October.
Whether your child will be receiving the vaccination or will not (a refusal), we require the completed form back for our medical records.
- If your child is receiving the vaccine: Please sign the consent section. Additionally, please supply your child’s original vaccination records.
- If your child is NOT receiving the vaccine: Please complete the refusal section.
If you have misplaced your form or need a new one, please contact the medical room and we will provide a replacement.
