Dear Parents,
Please read carefully the enclosed Department of Health’s invitation to join the School Dental Service and the Student Health Service. This is not a DBIS initiative, but parents may wish to take advantage of this offer. The appointment times for the annual check up will be outside school hours (from November 2022 to October 2023), and parents will be responsible for organising their own transport arrangements.
The School Dental Service Centre designated for our school is at Tang Shiu Kin School Dental Clinic, 5/F MacLehose Dental Centre, 286 Queen’s Road East, Wanchai, Hong Kong. The Western Student Health Service Centre designated for our school is at 9/F Rumsey Street Multi-Storey Carpark Building, 2 Rumsey Street, Sheung Wan, Hong Kong.
School Dental Service
The enrolment fee for Hong Kong residents to the School Dental Service is HK$30, and for non HK residents, the fee will be $725. For each application, whether you are a HK or a non-HK resident, please send along HK$30 and a copy of the relevant identity document. This joining fee of HK$30 should be paid by cheque to DBIS Ltd. as the Treasury will send the bill to the school. The staff of the Dental Care Service will contact the parents of non-HK residents for the payment of the difference in fee (HK$695) after receiving the application form.
Student Health Service
The Student Health Service is provided free to students who are HK residents or other eligible persons as noted on the form. For non-HK residents, the annual fee of HK$535 will have to be paid on the appointment day.
Please refer to the attached Notes for Parents/Guardians regarding guidelines to verification of eligibility.
Please return cheque and application form to the School Office on or before Monday 5th September.Â
